Congenital Spina Bifida  Defined  

Spina bifida (SPY-na BI-fi-da), a Latin term that means “split spine,” is a birth defect that occurs when a section of the baby’s spinal column does not form properly, resulting into a  neural tube defect (NTD) that occurs when a baby’s spine and spinal cord do not develop properly during the first month of pregnancy. 

The Pathophysiology

Failed Neurulation: The neural tube, which forms the brain and spinal cord, fails to close properly (usually between days 21 and 28 of gestation). This leads to an opening in the vertebral column, allowing the spinal cord and meninges to protrude.

The “Two-Hit” Hypothesis: Neurological disability in open spina bifida (myelomeningocele) is often viewed as a two-stage process:

First Hit: The failure of the neural tube to close.

Second Hit: The exposed neural tissue is damaged by direct contact with toxic amniotic fluid and mechanical trauma during gestation, leading to neuronal death.

 *Fluid Dynamics & Associated Malformations:*

 The disorder is associated with increased cerebrospinal

fluid (CSF) pressure within the central nervous system. This often leads to Chiari II malformation, where parts of the brain (hindbrain) are displaced downward into the cervical spine, causing hydrocephalus.

Neurological Dysfunction: The resulting nerve damage causes varying degrees of paralysis, sensory loss, and bladder/bowel dysfunction, typically occurring below the level of the lesion. 

Common Subtypes

Spina Bifida Occulta: Mildest form; vertebrae are malformed but the spinal cord remains protected under the skin.

Meningocele: Meninges protrude through the spinal opening, but the spinal cord remains in place.

Myelomeningocele: Most severe form; both spinal cord and meninges protrude, leading to significant neurological impairment. 

 *What are common complications of spina bifida?* 

Spina bifida can cause health problems for your baby, such as fluid on the brain, paralysis (not being able to move parts of the body) and learning and developmental disabilities.

 **Risk Factors* 

Folate Deficiency:* Inadequate folic acid intake is a major contributing factor.

 *Genetics & Environment: Genetic

factors* (e.g., Vangl2 gene mutations) and environmental influences such as maternal diabetes, obesity, and certain anti-seizure medications (e.g., valproate). 

 *Four Main Types* 

 *Spina Bifida Occulta:* The mildest and most common form. It is often called “hidden” because it is covered by skin and typically causes no symptoms.

 *Meningocele:* A rare type where a sac of spinal fluid bulges through an opening in the back, but the spinal cord remains in its normal position.

 *Myelomeningocele or Meningomyelocele* : The most severe form, where a sac containing parts of the spinal cord and nerves protrudes from the baby’s back, often leading to moderate-to-severe disabilities. 

 *Myeloschisis (or Rachischisis)* 

The most severe and rarest form within the “open” category. The spinal cord at the affected area is a flattened, plate-like mass of nervous tissue (neural placode) exposed to the environment without an overlying sac or skin.

This type is almost always associated with severe neurological impairment and other major anomalies like anencephaly (incomplete brain development).

 *Causes & Risk Factors* 

– **Folate deficiency** during pregnancy (Vitamin B9 is crucial).  

– **Family history** of neural tube defects.  

– **Medications** (e.g., anti-seizure drugs like valproic acid).  

– **Maternal diabetes or obesity**.  

– **High body temperature** in early pregnancy (fever, sauna, hot tub).        

      *Diagnosis* 

– **Prenatal tests**: Alpha-fetoprotein (AFP) blood test, ultrasound, amniocentesis.  

– **Postnatal signs**: Visible sac, skin markings, imaging (MRI, CT, ultrasound.  

    *Treatment & Management* 

– **Surgery**:  

  – Can be done **during pregnancy** (fetal surgery before 26 weeks) or **after birth**.  

  – Repairs the spinal opening and reduces complications.  

**Supportive care**:  

  – Physical therapy, mobility aids (braces, wheelchairs).  

  – Medications for bladder/bowel control.  

  – Shunts for hydrocephalus.  

– **Lifelong monitoring**: Specialized care teams follow patients through childhood and adulthood and promote periodic home visits after hospital discharge.- 

 Prevention 

Daily folic acid intake** (400 micrograms for women of childbearing age).  

Avoid overheating** during early pregnancy.  

Manage chronic conditions** like diabetes and obesity before conception.
